Support Worker
Program: Supported Living/Ongoing Support (supervised by Program Director)
Location: Community
Salary: $16.21-$22.82/hour (Non-Exempt)
Hours: Varies
Class: On-call
Support Workers who work an average of 30 hours per week for three consecutive months during the six-month measurement period are eligible for health insurance. Support workers receive five days of paid vacation benefits per year. Earned vacation is pro-rated based on the number of hours actually worked.
At the time of hire, temporary on-call employees will receive 24 hours of sick leave that can be used after 90 days of employment. If the employee has used any sick leave hours during the year, the total will be brought back up to 24 hours at the beginning of July each year.
Duties and responsibilities include the following. Other duties may be assigned.
- Implements objectives of people we support Individual Support Plans.
- Provides emotional support, nutrition awareness, house finding, and money management training to people we support.
- Utilizes community support services as appropriate.
- Provides transportation to various appointments.
- Maintains daily support notes.
- Communicates with support services program director, case managers, and other agencies as appropriate.
- Attends meetings and training as required.
- Lifts and transfers adult as required.
- Assists people we support with self-help activities such as feeding, toileting, dressing, personal hygiene.
- Assists consumers with special devices such as wheelchair, walker, prosthesis, or brace.
- Submits semi-annual progress reports on consumers.
- Performs other job-related duties as required.

EDUCATION and/or EXPERIENCE:
High school diploma or general education degree (GED) with a minimum of six months related experience with individuals with disabilities. College course work in the field of social welfare, sociology, psychology, education, or social services is desirable. Also requires knowledge of community resources.
LANGUAGE SKILLS:
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively in one on one and small group situations with consumers and their significant others, HCAR staff, the regional center, other agencies, and community members.
MATHEMATICAL SKILLS:
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
REASONING ABILITY:
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form.
CERTIFICATES, LICENSES, AND REGISTRATIONS:
First Aid and CPR certification, TB and fingerprint clearance, and physical exam.
OTHER SKILLS AND ABILITIES:
Ability to prioritize job responsibilities and relate to individuals with disabilities. Ability to effectively support consumers in finding suitable housing, relocating, budgeting, shopping and managing household. Reliable transportation is also required. All employees must possess the ability to work with a cheerful and caring attitude toward their coworkers, people we support and the community.

Life Skills Coach
Program: Bay Center Day Services (supervised by Division Coordinator, Program Manager, or Site Director)

Paid vacation, sick leave, and holidays according to personnel policies. Health insurance is provided for regular employees scheduled to work at least 30 hours per week within 90 days of employment.
Duties and responsibilities include the following. Other duties may be assigned.
- Implements people we serve with comprehensive Individual Service Plans under the direction of the Program Manager.
- Participate in people we serve assessments and ongoing evaluation of Individual Service Plan Objectives and Behavior Support Plans.
- Keeps timely records (daily tracking, case notes, etc.) of targeted tasks and behaviors for Individual Service Plans and Behavior Support Plans.
- Provides care and supervision while supporting people in skills training activities relating to individuals and groups in community-integrated settings. Skill training includes behavioral/social skills, community resources, independent living, and
vocational skills. - Provides people we serve with skills training to reduce or re-direct negative behaviors and promote and reinforce positive behaviors using verbal and nonverbal prompts.
- Supervises people we serve in vocational skills training and maintains their timesheets for paid jobs.
- Assists people we serve with personal care activities such as toileting, feeding, and personal hygiene.
- Assist people we serve with personal devices such as a wheelchair, walker, prosthesis, or brace.
- Lifts or transfers adult people.
- Lifts or moves objects weighing up to 50 pounds.
- Drives vans with people we serve to various community locations as required.

CERTIFICATES, LICENSES, AND REGISTRATIONS:
Current CPR and first aid certificates, certification of TB clearance, physical exam, and fingerprint clearance. Class B California Driver’s License may be required.
